CASE: THE BLOOD SUPPLY OF THE HEART

The city was ringing in the new year. Midnight saw Titaly and her granddaughter Rebekha arrive at the hospital. 69-year-old female Due to persistent chest pain, Mrs. Titaly K sought emergency medical attention.

Titaly's chest pain persisted for a while in Jan 2010, and she went to the doctor at that time. After being brought to the hospital, Titaly was given the diagnosis of myocardial infarction.


Titaly smoked continuously and had hypertension, diabetes mellitus, dyslipidemia, and dyslipidemia. During the course of Titaly's recovery from the myocardial infarction, she underwent coronary angiography, which revealed the presence of lesions in the right coronary, anterior descending, and circumflex arteries that were 70% obstructed.

Doctor Abraham attended the case and reviewed the whole report; left ventriculography revealed apical akinesia with intracavitary thrombus symptoms.

The Doctor was explaining it to Titaly's granddaughter Rebekha - It is a kind of Coronary artery disease (CAD). The daughter Rebekha was enquiring with interest. What is it, and how did it occur? The doctor began by addressing the question by saying, "The blood flow of the heart is affected. I'll explain it to you."

Dr. Abraham: The heart is nourished by the right and left coronary arteries. The right coronary artery originates in the anterior aortic sinus. Prior to running in the anterior atrioventricular sulcus to the junction of the right and inferior boundaries, it descends between the pulmonary trunk and the right atrium.

Rebekha: Was listening carefully; Doctor continued - At this point, it turns to run in the right posterior atrioventricular sulcus, crosses the crux, and then anastomoses with the circumflex branch of the left coronary artery to come to an end. Branches: The anterior and posterior portions of the right atrium are supplied by atrial branches. The nodal branch, which serves the SA node, is one of these branches.

The right ventricle's anterior and posterior regions are supplied by ventricular branches. One of these branches, known as the right marginal artery, goes along the inferior border. The posterior interventricular artery supplies the right and left ventricles' adjacent regions as well as the posterior third of the interventricular septum, running in the posterior interventricular groove.

The left posterior aortic sinus is the origin of the left coronary artery. It briefly extends in the left anterior atrioventricular sulcus before giving rise to the anterior interventricular branch and circumflex branch.

The doctor double-checked with Rebekha, "Did you understand what I was saying? Rebekah retorted, "Hmm." The doctor went on... Doctor: The anterior interventricular branch runs in the posterior interventricular grove, where it anastomoses with the posterior interventricular artery, after descending in the anterior interventricular groove and circling the inferior edge. It provides branches that feed the anterior 2/3 of the interventricular septum as well as the anterior and posterior portions of the right and left ventricles.

The circumflex artery travels around the left edge before ending by anastomosing with the right coronary artery at the left posterior atrioventricular sulcus. The left atrium and ventricle's posterior regions are supplied via its atrial and ventricular branches.

The left marginal artery is the name of one of the ventricular branches that descends on the left margin. Vascular drainage;

The primary venous drainage system for the heart is the coronary sinus. It begins as a continuation of the great cardiac vein at the left end of the posterior left atrioventricular sulcus. It opens into the right atrium's posterior smooth section as its final entrance. The Great cardiac vein, Left atrial oblique vein, Left ventricle posterior vein, Middle cardiac vein, and Small cardiac veins are its tributaries. It originates from the sinus venous' left horn.

Due to a medical emergency, the nurse summoned the doctor. The doctor said to Rebekha, "We'll talk more later." I must now go to another patient.
